Affiliate Program Contract Request Form

Please complete this form to initiate contract preparation for any new SAFM Affiliate Program member. SAFM Operations reviews all requests within 24 hours of submission Monday through Friday. If further clarification is required, a 15-minute phone consultation will be scheduled.

To ensure all required paperwork is secured on time, you are encouraged to submit requests, at least, 45 days in advance of any target event. This minimum timeline supports not only SAFM resource planning but also the often needed buffer required to allow for delayed guest responses or legal review of the associated paperwork. Submissions received outside of this timeframe will be reviewed on a case by case basis to determine appropriate delivery timelines.
