All case content must be submitted to the certification administrator prior to the end of (and not beyond) the respective Level 3 examination window. The Level 3 AFMC exam will not be evaluated until all components of the case study have been submitted. Once submitted electronically via this page, case study documents may not be edited or withdrawn. Emailed submissions will NOT be accepted.
A complete Case Study submission must include each of the following items in response to the SAFM Case Study and the data shared about the current case covered in the AFMC Level 3 Exam Competency Case Webinar and Intake Form:
(1) An SAFM Map which you may populate via PowerPoint or PDF format. All details of the map must be captured, legibly, on a single page which can be printed to a single, 8.5″x11″ piece of paper.
- To fulfill the specifics of your exam, and as part of your grade, you will be required to assess the client and case details, and submit an SAFM Map displaying (1) your initial client and case findings and then, (2) as a secondary action, displaying the interconnectedness you see, along with suggestions, questions, and anticipated next steps. You may print out and complete the map by hand or use a fillable, computer-generated copy. Both the PDF and PowerPoint SAFM Map versions are available for your use and located in the AFMC Level 3 Exam course, under Exam Templates. Either format must be uploaded as a PDF in its final form.
- Remember, the goal of the Case Map is to demonstrate (a) your competency and understanding of the client and the case through the functional medicine lens and (b) your ability to deliver that information in a clear and concise way on the Map.
- The Map Submission for the Exam has very specific requirements:
- Part 1 – Using BLACK ink (or black font on the computer generated copy) on the Map, record the details of the case as they have been presented to you. When you are finished with Part 1, move on to Part 2.
- Part 2 – Using RED ink (or red font and arrows on the computer generated copy) on the Map, record what questions might be raised for you regarding the details of the case, where you see areas of interconnectedness, and any suggestions you would want to explore further (i.e. lab work, dietary changes, lifestyle changes, etc.).
- Your Map should clearly show accurate details of the client and the case dynamics, and should be legible to the examiners in its final and uploaded PDF format.
(2) A Case Assessment, which is a questionnaire with specific questions that you will answer to demonstrate your interconnectedness competency.
- Make sure you have all of your case materials at hand and ready for reference to complete this assessment. There is no time limit within which you must complete this form. However, you will need to complete all of the following questions in a single sitting and submit them all at once at the end of the page.
- The entry field for every question has a purposely imposed character limit. This limitation requires you to prioritize the details of your case and express yourself succinctly (a key practitioner skill).
- CAUTION: Plan sufficient time to document specific, detailed insight on and thorough explanation for each of your points. Vague generalities will not be accepted. We recommend creating multiple revisions of you assessment with keen progressive improvement prior to your exam submission to ensure your best showing on the exam.
- Remember, the goal of the Case Assessment is to thoroughly and competently demonstrate the breadth and depth of your your functional medicine knowledge. Be comprehensive. We look for each of your answers to be unique to each other.
- Again, in the interest of clarity and due to the nature of the Case Study Assessment, we cannot over-emphasize these rules. Please do not 1) share any case details whatsoever with others and do not 2) collaborate, study or prepare together in any way whatsoever.
