SAFM Activator – Application

Thank you for your interest in being part of the SAFM Activator program!

Registration for this program is open for all SAFM students after completing the AFMC Level 2 exam. Membership is by application only for students who have so far completed only the AFMC Level 1 exam.

We know that completing SAFM’s applied functional medicine curriculum is an ongoing commitment and major time investment. We want to be sure that students are not investing in more education and resources than they are able to reap value from at once.

If you have not yet passed the AFMC Level 2 exam but believe you would be a great fit for Activator at this time in your SAFM journey, please kindly complete and submit this entire application for consideration.

Note: Application status will be communicated to all applicants by end of day Monday 22nd at the latest.
