Uploaded documentation must:
- Show your full name
- State the expiration or renewal date (if applicable to your licensure or certification)
- For licensed practitioners – validate your legal capacity to practice your modality in your state/country. Submit a copy of a physical license from your State board or this may be in the form of a screen shot of your Board’s online licensure verification report.
- For non licensed practitioners – submit your modality completion certificate.
A degree diploma or program syllabus will not serve as sufficient validation.
