SAFM Platinum Inner Circle Retreat Participant Agreement (Spring) – Full Page

I understand that my registration herein makes me a Participant in the Platinum Inner Circle Retreat, an event produced by Purpose Inc and implemented in partnership with several independent contractors (herein “Retreat Workers”).

I understand that Purpose Inc promises to provide, on a best-effort basis, the full set of activities planned for this event. I understand that weather, technical difficulty, transportation failure, building malfunction, or any other outcomes, foreseen or unforeseen, may require a change in agenda, schedule, activities, or accommodation. I understand that I am fully responsible for the coordination and cost of my transportation to and from the retreat facility.

I understand that Purpose Inc, Retreat Workers, and the event venue do not assume any responsibility for injuries resulting from my failure to use due caution.I understand Purpose Inc is not responsible for any accidents or injuries to me in transit to/from or during this event. I assume all liabilities for my own well-being and will not hold Purpose Inc or the Retreat Workers or the retreat venue/facilities liable in any event. I understand that meals during the retreat will be prepared on a best-effort basis to be absent of gluten- and dairy-containing ingredients, but acknowledge that the event venue does not operate a gluten/allergen-free facility.

I take full responsibility for deciding my appropriate level of participation in all retreat activities. Given the limited nature of interaction during this event, I understand that bodywork service providers and exercise facilitators (i.e., Retreat Workers) are not able to monitor or provide recommendations for me as an individual. I acknowledge that Purpose Inc, Retreat Workers, and the retreat venue hold no responsibility for monitoring any medical condition I might have, with or without prior disclosure, including allergies. I understand that, while participating in retreat activities, I could sustain bodily injury and am nevertheless voluntarily agreeing to participate.

I hereby release, indemnify, protect, and hold harmless Purpose Inc, all participating Retreat Workers, and the retreat venue and all of its employees from any and all claims, demands, personal injuries, costs or expenses, arising out of my participation in this event. I acknowledge that by participating in this event, I assume full responsibility and liability for any and all payments due to Purpose, Inc, per written request, for any damages to the retreat venue property caused by my misuse, negligence, or damage, inadvertent or otherwise.

I understand the following with regard to cancellations and refunds.Cancellation Policy:
- Cancellations must be made in writing via email to the PIC Membership Coordinator (Sarah@SchoolAFM.com) and are effective as of the email time stamp of receipt. We recommend you follow up promptly to ensure no unexpected delivery issues.
- Cancellations received by 11:59 pm ET on February 14, 2024 will receive a full refund minus a $50 credit card cancellation fee.
- Cancellations received between February 15 and 29, 2024 by 11:59 pm ET will receive a full refund minus a $200 credit card and administrative fee.
- Cancellations received on or after March 1, 2024 will not be eligible for any refund. Registration may be transferred to another PIC member desiring to attend the retreat. Coordination of any retreat registration transfer must be initiated, in writing, by the original registrant via email with a copy to the alternate registrant and the PIC Membership Coordinator. Transfers not confirmed by the dates outlined herein remain the financial responsibility of the original registrant.
All retreat fees are nonrefundable after March 1st, 2023. I understand no refund will be available in the event I am unable to participate in any portion of the retreat, unless there are extenuating circumstances that have been presented to, discussed with and approved by the Platinum Inner Circle Membership Coordinator. Examples of this include but are not limited to death and/or legitimate family emergency and/or major illness.

I understand my likeness or voice may be captured in retreat photographs or video recordings in public areas of the retreat venue, and I give my permission for full, unrestricted use of these materials by Purpose Inc for sharing memories with attendees, marketing, communication, and follow-on program purposes.

I understand that my participation in this event is dependent on agreement with this Release. I have read this agreement thoroughly and have been given the opportunity to ask any questions. I have received and understand the information which was provided.
